Our Office Policies
We look forward to seeing you on your scheduled appointment date. Our office may contact you a few days prior to your appointment to confirm the appointment date and time, pre-register standard information and remind you of the upcoming appointment. When checking-in, it may be necessary to obtain copies of insurance ID card(s) in order to bill your insurance company.
Appointments/Cancellations
With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.
Emergencies
My office provides on-call service for emergencies. You must call so that the appropriate provider on-call can be contacted. After office hours, an answering service may take your message and immediately contact a provider with your information.
Note: It is not possible to schedule emergency appointments online.

Costus Medical & Pharmacy provides our patients with office forms for your convenience. We recommend you print and fill these forms prior to your appointment to save time.
